Should the owner of the business, appear personally for a business name registration?
- The presence of the owner of the business along with his NIC is compulsory for this purpose.
What are the documents to be produced in the procedures related to the Bank Savings and mortgaged property of a deceased person?
- Death certificate of the deceased person.
- Documents in proof of the relationship of the deceased person to the claimant (Birth certificate/ marriage certificate).
- Affidavits from other parties (if any) declaring no objection to transfer money or goods to the claimant.
- National Identity Card.

How to obtain the allowance of Rs. 20 000/- issued for the pregnant mothers?
- Before your pregnancy exceeds 3 months, you can obtain the relevant application form from the early childhood development officer of your Divisional Secretariat that your Maternity Clinic is registered or from the officer in charge of the programme.
Who is eligible for obtaining the education aids?
- The schooling children of a deceased or a permanently disabled father are eligible.
How a NGO can be registered?
- A NGO founded within not less than a year can be registered after submission of the relevant documents.
Is it possible to make new registrations for the Samurdhi Scheme?
- Currently, New beneficiaries are not being selected and only the appeals are accepted to be considered in the future.

How the copies of the Birth / Marriage/ Death Certificates can be obtained?
- Only if the number and the issued date of the Birth/Marriage/ Death Certificate is known an amount of Rs 100/- and if the number and the issued date is not known and amount of Rs. 200/- should be paid to the Divisional Secretariat and thereafter the copies are issued.
- Only if the Birth/Marriage/ Death had taken place within the Kegalle District, the copies of the respective certificates can be obtained from the Kegalle Divisional Secretariat.
How the Births can be registered?
- Registration of a Normal Child Birth can be done by the Registrar of the relevant division, before 3 months after the birth.
- For the Child Births exceed a time lap of 3 months, contact ADR for more details.
Pension Division
How to receive the W&OP after the death of the civil pensioner?
- Firstly the death certificate of the pensioner should be obtained and the pension salary must be withholded.
- If an excess amount is available to inform the bank and get it returned.
- inform the person to bring the following documents

- Original of the Death certificate of the pensioner.
- Original of the birth certificate.
- Original of the Marriage certificate.
- A photocopy of the widow's bank passbook.
- A photocopy of the ID of the widow.
- A photocopy of W&OP pension card.
- a passport sized colour photograph, certified by the Grama Niladhari.
- All the above documents must be certified by the Grama Niladhari.
- Take necessary actions to complete in w&op pension procedure.
Why the W&OP pensions has been suspended?
- Life certificate for the respective year has not been submitted.
- The pensioner has been asked to submit the life certificate after Grama Niladharis certification.
- After resubmission of the life certificate W&OP pension should be updated.
